If your home address, telephone number, GP, pharmacy, or health insurance details have changed, please let us know, preferably with this form.
- Remember to include your patient number in the correspondence.
- Do not send us any personal medical information with this form.
You can also have your personal details changed at the Registration Desk in the hall, or you can change your own personal details, phone number, email, and the details of your GP and pharmacy at Mijn Erasmus MC.
