How to complete the questionnaire
You will receive an invitation by email to complete the questionnaires a week before your appointment.
Step-by-step guide
You will receive an invitation for the questionnaires by email.
Follow the steps:
- Follow the link in your e-mail or go to www.erasmusmc.nl/mijnerasmusmc.
- Log in with your DigiD.
- Do you need to complete questionnaires for your child? Log in using your child’s DigiD.
- Do you want to log in on behalf of someone else? Then request authorization first by going to https://machtigen.digid.nl/.
- If you don’t have a DigiD yet or have forgotten your password, go to https://digid.nl/aanvragen. It will take about three working days before you receive your DigiD or a new password.
- Click on ‘Questionnaires’ (or ‘Vragenlijsten’ in Dutch) to complete the questionnaires that are available for you. Fill them in at home before your appointment.
- You will discuss the answers with your care provider during your appointment.

Via the Digizorg app
Do you not have the Digizorg app yet? Download it from the Play Store or the Apple Store.
You will receive a notification from the Digizorg app when there are questionnaires for you to complete.
Follow the steps below:
- When you have opened the app, click on ‘Dossier’ in the bottom right corner
- Click on the notification indicating that there are questionnaires to be filled in
- Click on the section ‘Questionnaires’ (or ‘Vragenlijsten’ in Dutch)
- Look under the heading ‘To do’ (or ‘Te doen’ in Dutch)
- Click on the questionnaire and fill it in
Care that is tailored to your needs
During your treatment, you may be asked to complete one or more questionnaires. This will give an impression of how you are doing during the treatment. For example: are your symptoms improving or getting worse over time? Are you suffering from fatigue or pain? Can you do everything you want in your daily life? It will also give you the opportunity to let us know what you wish to discuss during the appointment.
This will enable the healthcare provider to tailor the conversation better to your situation. It will make it easier for you to discuss what your wants and needs are. That way, you can decide together on the care that suits you best. The questionnaire(s) facilitates this process.
Example: For Mr. de Bruijn, music is the most important thing in his life. He needs to undergo surgery, but this will cause his hearing to deteriorate. If the care provider knows that music is very important to Mr. de Bruijn, they can discuss whether other treatment options are available.
What do we do with the answers you give?
Your answers will be used to provide care that is tailored to your needs
You will discuss your answers to the questionnaire with your care provider. This will allow us to tailor the care even more to your personal situation. Together with your care provider, you will choose the best possible treatment.
Your answers will help medical research
The information that you provide is also very important for medical research into the best care for patients who have the same medical condition.
The completed questionnaire will help healthcare providers understand the course of a disease or condition in patients. If a patient is unsure about whether their symptoms are normal, the healthcare provider can answer their questions. They can also show which treatment options patients are most satisfied with. We can continue improving healthcare by comparing medical research with questionnaire outcomes. And in the future, we will be able to better predict which treatment is the best choice for a patient.
Giving your consent
We will therefore ask you whether we can use the answers you give and the information about your treatment (hereinafter: "your data") for future studies. If you consent to us using your data, we will do so in an encrypted and anonymized form (without your name). This means that researchers cannot see which data belong to which patient.
Whether you give or don't give consent will not affect your treatment in any way.
Withdrawing consent
You are free to withdraw your consent to use your data at any time. This will have no adverse effect on the treatment provided. To withdraw consent, go to ‘Tasks’ in the secure My Erasmus MC patient portal.
Even if you do not consent to the use of your data for research purposes, we would still like you to complete the questionnaire.
This will help you to prepare for your appointment and enable you to discuss your answers with the healthcare provider.
National quality registers
There are a number of quality registers in the Netherlands, including at the national level. These registers are a collection of (anonymous) medical data of patients and their experiences with various treatment methods. This allows insight into the outcomes of a treatment and enables treatments to be improved on where possible. The data in quality registers are secure and cannot be directly traced back to you as a person. Erasmus MC participates in various quality registers.
